What is an OFI?

OFI stands for Opportunity for Improvement. It is a standard form utilized to improve patient care or systems within the organization.

When and What To Report

Something that is not consistent with the patient routine care or with accepted standard of care.

A unique event.

Incidents could be a bad or unexpected outcome, but should not always be equated with an error or mistake.

Helps us identify and monitor opportunities for improvement in processes or systems.
